Student Requests

Questions on how to make requests for transcripts or letters of recommendation? Make an appointment with your counselor via email or stop in the SD office during your lunch.


Transcripts for Scholarships, College Admissions, and Enrichment Programs may be requested in the SD office and online through your Naviance account. Students must give their School Counselor one week to process their request. To log into Naviance follow the steps below:

  • Naviance Login Page
  • Your username is your CPS username.
  • Your password is your 8 digit ID number.

* All transcript requests for College Admissions must be requested via the paper request and handed in to your counselor. Transcript requests will not be sent until the college application is completed.

Recommendation Letters

Recommendation letters must be requested in person and on Naviance. Make sure to hand your potential recommendor a completed Goode Brag Sheet when you are requesting a document. You must give staff at least 2 weeks to complete a recommendation letter for you. We recommend that you have at least two teacher letters of recommendation. If you are using the Common Application be sure to notify your recommendor that they have to complete the Common Application Teacher Recommendation Form in addition to their letter.
