4. Provide your information

5. Confirm the information that you inputted

6. Download and print/take a screenshot of your ticket

VACCINES

Pfizer COVID-19 (≥ 5yo - 2 dose series over 3 weeks) **

Moderna COVID-19 (≥ 18yo – 2 dose series over 4 weeks)

Janssen/Johnson & Johnson (≥ 18yo – 1 dose series)

COVID-19 Booster (clinician review required) **

The COVID-19 vaccines are FREE and available to all eligible Chicagoans, regardless of insurance or immigration status.

*Second dose appointments:
